In order to play a sport students need:
1. Get a Pre-participation Physical Evaluation. We have a form here but any doctor's form that states the student is cleared for all sports is acceptable.
2. Register on HomeCampus
3. Show up to tryouts on the day/time outlined here.

Online Clearance 

Online clearance must be completed for each sport you intend to participate in this year.

  1. Go to athleticclearance.com

  2. Complete the Log-in Process.

  3. Complete the Online Forms.

  4. Complete the Online Signature Agreements. *

* - If purchasing Student Insurance, please turn in a copy of the printed ID card to the Athletic Training Room

Physical Form

Physicals are required annually and must be valid throughout the entire season of your sport.

 

  1. Download the Form - CLICK HERE

  2. Complete the top portion prior to doctor visit. If you cannot print or have a physical on a different form, that is ok.  We will accept athletic physicals on the doctor's office form.

  3. Have a licensed medical doctor (M.D. or D.O.), N.P., or P.A. complete, sign and stamp the bottom portion of the form.

  4. Upload a copy of current physical to the athleticclearance.net website or bring it directly to the Athletic Training Room.

Athletic Charter

No bills or fundraising requests will be approved until steps 1 & 2 are complete.

   Wet Bulb Globe Temp. (WBGT)

Activity Guidelines

   Less than 79.7F

Normal activities - Provide at least 3 separate rest breaks each hour with a minimum duration of 3 min each during the workout.

   79.9 - 84.6 F

Use discretion for intense or prolonged exercise. Provide at least 3 separate rest breaks each hour with a min duration of 4 minutes each.

   84.7 - 87.6 F 

Maximum outdoor practice time is 2 hours. Provide at least 4 separate rest breaks each hour with a min duration of 4 min each. For Football: Players are restricted to helmet, shoulder pads, and shorts during practice. If WBGT rises to this level during practice, players may continue to work out wearing full pads without changing to shorts.

   87.8 - 89.6 F

Contests are permitted with additional hydration breaks. Maximum outdoor practice time is 1 hour. No protective equipment may be worn during practice and there may be no conditioning activities. There must be 20 minutes of rest breaks distributed throughout the hour of practice.

   Greater than 89.8 F

No outdoor workouts/contests. Delay practice/competitions until a cooler WBGT is reached.

   Air Quality Indicator (AQI)

Activity Guidelines

   0-50

Air quality is satisfactory and poses little or no risk.

   51-100

Sensitive individuals should avoid outdoor activity as they may experience respiratory symptoms.

   101-150

General public and sensitive individuals in particular are at risk to experience irritation and respiratory problems.

   151-200

Increased likelihood of adverse effects and aggravation to the heart and lungs among the general public.

   201-300

General public will be noticeably affected. Sensitive groups should restrict outdoor activities.

   301+

General public at high risk of experiencing strong irritations and adverse health effects. Should avoid outdoor activities.
